Home Health Care of Lima Memorial works together with patients, families, caregivers and physicians to help individuals maintain independence at home after being discharged from a hospital or long-term care facility.
Our experienced home care staff travels to individual homes, delivering care for all generations. Registered Nurses, Therapists and trained Home Health Aides instruct the patient and family how to care for his or her specific health needs through continued care, education, treatment, intervention and monitoring. In addition, Lima Memorial’s Home Health Care staff will work with your physician and insurance company to arrange Home Health Care services
Home Health Care is available in Allen, Hardin, Auglaize, Logan, Putnam, Van Wert and Mercer counties.
